He was born April 9, 1856 in Elkfork, Morgan County, Kentucky, died September 21, 1943 in West Liberty, Morgan County, Kentucky, and is buried in Hill Cemetery in Dingus, Morgan County, Kentucky.

According to her death certificate, Philadelphia Conley Blevins died November 28, 1918 in Morgan County, Kentucky, from influenza and lobar pneumonia. Also on this certificate, her birthplace is listed as Morgan County, her birth date as November 28, 1918 [mistake], her age as 61 years 8 months and 4 days, her marital status as married, her parents as Andrew J. Conley (b. Kentucky) and Lou C. Patrick (b. Magoffin County), the informant as J. T. Blevins of Dingus, Kentucky, and the burial place as Perlina Williams Cemetery. Although Delphia died in 1918, there is a 66 year old Delpha Blevins listed as the wife of John Blevins in the 1920 census.
